🦴 PRP與BMAC在骨折癒合中的應用：我會適合嗎？

感謝中華民國骨科醫學會邀請，2025年4月19日吳醫師至嘉義分享骨折後使用再生醫學藥物的經驗，會議過程討論熱烈，茲將演講過程摘錄如下。

骨折的癒合通常需要數週甚至數月，但在某些情況下，癒合速度不如預期，甚至出現「延遲癒合」或「骨折不癒合」的情形。這時，再生醫學的介入，如PRP（高濃度血小板血漿）與BMAC（濃縮骨髓抽取液），有機會成為加速癒合的突破口。

🔍 什麼是延遲癒合與骨折不癒合？

• 延遲癒合（Delayed Union）：骨折在合理時間內尚未出現癒合徵象，通常超過3個月。

• 不癒合（Nonunion）：骨折在6個月後仍無明顯癒合，甚至無進展。

🧪 PRP與BMAC能幫助骨折癒合嗎？

根據最新的臨床研究與文獻統整：

✅ 對延遲癒合或不癒合有效

• 2024年最新系統性綜述（Umbrella meta-analysis）指出，PRP可顯著縮短癒合時間（平均提早1.25週），並提升癒合成功率（RR = 1.30）。

• 結合BMAC與PRF（血小板纖維蛋白）的手術補強，也明顯加速上肢骨折癒合，尤其是肱骨、橈骨、尺骨等部位。

• 經皮注射BMAC+PRP的方式亦在中國大型RCT中證實可提升延遲癒合患者的癒合進展。
  
  

⚠️ 對急性骨折的幫助有限

• 幾篇針對急性股骨幹骨折與新鮮頸骨折的隨機試驗顯示，PRP組在癒合速度與成功率上未顯著優於控制組。

• 然而，針對特定高風險骨折類型（如Jones骨折、舟狀骨等容易延遲癒合或不癒合的部位）的小型研究，顯示PRP可能縮短運動員復原時間、減少復發。
